1. Application for Music Lessons
1.1 Personal Information: The applicant agrees to provide accurate personal information, including name, surname, date of birth, age, email address, home address, and telephone number.
1.2 Consent for Publicity: The applicant grants permission to be included in group photographs, video shoots, and concerts for publicity purposes.
1.3 Choice of Instrument and Schedule: The applicant selects one or two instruments and preferred days for lessons according to the provided options.
2. Details of Guardian Responsible for Fees
2.1 Guardian Information: The guardian responsible for fees agrees to provide accurate information, including name, surname, telephone number, and email address.
3. Additional Contract Terms:
3.1.1 Interactive Sessions: Students will attend fun and interactive sessions with the tutor and interact with other students of similar passion.
3.1.2 Time Slots: Time slots for lessons will be communicated to the students.
3.1.3 Grades: There are eight grades (levels) to be completed. Beginners are considered Grade 1, and progress will be assessed in the third week of December each year.
3.1.4 Lesson Structure: Lessons are one hour in duration (30 minutes of theory and 30 minutes of practice), and students are expected to take extra work home.
3.1.5 Activities and Trips: Information about activities and music field trips will be made available to the students.
3.1.6 Tutor Responsibility:Tutors will not be responsible in the absence of a student to work additional hours. Students are required to commit to their given time and be available for classes.
4. Payment Agreement
4.1 Registration Fee: A non-refundable registration fee of $100 is required for all instruments.
4.2 Monthly Fees:
4.2.1 Payment to be made on or before the 3rd of each month. An additional 5% will be added to the amount if not paid.
4.2.2 Primary Students (Age 4-12): NAD 450 per month in addition to the registration fee.
4.2.3 Secondary Students (13 and above): NAD 550 per month in addition to the registration fee.
4.2.4 Working Class: NAD 580 per month in addition to the registration fee.
4.2.5 Dance Choreography: $550 per month for all ages.
4.3 Payment Schedule: Payments are to be made on a monthly basis for the duration of one year to complete Grade 1 (Level 1 students).
4.4 Mid-Month Joining:
If a student joins in the middle of a month, a half payment will be required, and full payment will commence the following month.
4.5 Penalty Fee:
A penalty fee of NAD 700 will be required if a student decides to leave before completing the agreed-upon time period. Consistency, commitment, dedication, and effort are expected from the student.
5. Dispute Resolution
5.1 In the event of any dispute arising between the Music Academy and the student, parent, or guardian, both parties agree to engage in good faith negotiations to resolve the dispute amicably.
5.2 If the dispute cannot be resolved through negotiations within 30 days of written notice from one party to the other, both parties agree to participate in mediation.
5.3 The mediation will be conducted by a mutually agreed-upon mediator or, in the absence of an agreement, a mediator appointed by a recognized mediation service.
5.4 Each party shall bear its own costs related to the mediation process.
5.5 If mediation is unsuccessful in resolving the dispute, either party may pursue other legal remedies available under applicable laws.
5.6 Any legal action or proceedings arising out of or relating to this agreement shall be brought in the competent courts of the jurisdiction where the Music Academy is located, and both parties consent to the jurisdiction of such courts.
6. Miscellaneous
6.1 Any modifications or amendments to this agreement must be in writing and signed by both parties.
6.2 This agreement constitutes the entire understanding between the parties and supersedes all prior agreements, whether written or oral.
6.3 The headings in this agreement are for convenience only and do not affect its interpretation.
By proceeding with registration, both parties acknowledge that they have read, understood, and agreed to the terms and conditions outlined in this agreement, including the dispute resolution clause.
